The State of Colorado approves the following conditions for Medical Marijuana:
- Cancer
- Glaucoma
- HIV or AIDS positive
- Cachexia (loss of appetite, weight loss, muscular wasting and general mental and physical debilitation, usually due to advanced disease)
- Severe or chronic pain (E.g. degenerative disc disease, carpel tunnel, arthritis, migraines, Asthma, Neuropathy, Fibromyalgia, Crohn’s Disease, GERD, Hepatitis-C)
- Severe or chronic nausea (including due to medications)
- Seizures (E.g. Epilepsy)
- Persistent muscle spasms (E.g. spastic disorders, Multiple Sclerosis)
How to get your card:
- Download the Get Your Card packet.
- Complete the Medical Marijuana Registry Application for Identification Card. It must be notarized so wait to sign the application in front of the notary.
- Your physician must complete and sign the Physician Certification form.
- A legible photo copy of a photo ID that establishes Colorado residency for the patient
- Non-refundable application fee of $90 (check or money order payable to CDPHE)
- Send all of the above to the Colorado Department of Public Health and Environment at the address on the forms, certified mail.

Helpful Tips:
- The cheapest and fastest way to get your physician’s certification is to have your primary care physician, who is already familiar with your condition, complete and sign your Physician’s Certification form loacted inside the Get Your Card packet.
- If you decide that you would like to keep your Certification out of your primary care physician’s file and would like a different doctor to provide your Certification, a list of recommended medical clinics is in the sidebar to the left.
- It is usually easiest and cheapest to obtain your medical records from your primary care physician to substantiate your condition with the new doctor, rather than repeating the required tests.
- Specialized Medical Clinics usually require that you fax them copies of your recent (less than 3 years) medical records verifying your qualifying medical condition. After reviewing your records they usually call you to set up an appointment so that you can meet a doctor and discuss your condition. The doctor then completes your Certification and the paperwork is usually notarized at the clinic. These clinics can charge you anywhere between $90 and $200 for this service.
- If you require a full examination and/or tests to substantiate your condition, many clinics can provide this service for additional charges.
